The past decade has highlighted the need to modernise the delivery of fast, responsive and effective Emergency and Urgent Care, providing a wider range of services appropriate to patients needs and transforming the way Unplanned Care services are delivered.

The development of the Advanced Practitioner role was established as a key element to the delivery of this transformation enabling a flexible and highly autonomous workforce. 

The focus was placed on breaking down traditional professional boundaries, which is vital to ensure patients receive the highest standard of care.  This care should be delivered by an appropriately trained person, at an appropriate time and in the most appropriate setting - Right Skill, Right Time, Right Place - this is fundamental to the success of an effective Unplanned Care Strategy.

Our innovative approach has already been recognised by the Health Service Journal as SSAFA Hallam Care LLP has been shortlisted for the Acute & Primary Care Innovation Award at the 2010 HSJ Awards.
